- W2061058801 abstract "To evaluate whether endometrial thickness (ET) in women diagnosed with pregnancy of unknown location (PUL) can be used as an indicator of potential likelihood of outcome. This was a prospective observational study of consecutive pregnant women attending an early pregnancy unit. ET was measured using transvaginal ultrasonography in women diagnosed with a PUL at first presentation over the study period. Women were followed up until the final outcome was available. Likelihood ratios (LR) were calculated correlating ET (at given gestational age) and the outcome of the pregnancy, this being a failing PUL, normal intrauterine pregnancy (IUP), ectopic pregnancy (EP) or a persisting PUL (PP). ET was measured in 993 women who initially presented with a PUL. Of these 529 (53.3%) were subsequently diagnosed as failing PULs, 374 (37.7%) had IUPs, 73 (7.4%) had EP and 17 (1.7%) as PP. An ET of 0–5 mm was found to be suggestive of a failing PUL with a positive LR of 4.78 and relative risk (RR) of 0.84 compared to an ET of 15.1–17.5 mm being suggestive of an IUP (LR 3.37, RR 0.67). In women subsequently found to have EP and PP, an ET of 5.1–7.5 mm was found to produce the highest likelihood ratios, this being 1.18 and 2.36 (RR 0.09 and 0.04) respectively. Measuring ET could be used as a potential predictor of outcome in women who present as PUL. A thin endometrium is more likely to be associated while a thicker endometrium is more suggestive of an IUP. Using ET to predict the outcome of EP and PP may be less reliable." @default.
