• W2061570394 abstract "重症の救急患者にみられるショックや臓器障害の病態は侵襲に対する生体反応に起因し,その発生機序にサイトカインが重要な役割を演じている。サイトカインはエンドトキシンなどに対しマクロファージ,単球,リンパ球,好中球などが産生する生理活性物質で,多くの種類がある。腫瘍壊死因子(TNF),インターロイキン(IL)-1, IL-6, IL-8などは炎症性サイトカインと言われ,とくにTNF-αとIL-1とは侵襲直後に産生され,他のサイトカインや種々のメディエータの産生を誘導する。エンドトキシンなどの外因を投与しなくても,TNF-αやIL-1-βの投与によりショックや臓器障害が起こることは,外因に対する生体反応がショックや臓器不全を起こすことの証拠として重要な知見である。IL-6は急性相反応のalarm hormoneの役割を演じ,IL-8は好中球の走化・活性化因子としてARDSの病因となる。一方,IL-4, IL-10,可溶性TNFレセプター(TNFsr)やIL-1レセプターアンタゴニスト(IL-1ra)などは抗炎症性サイトカインであるが,侵襲下では炎症性サイトカインと抗炎症性サイトカインの両者とも産生が亢進する。サイトカインの多くはautocrineやparacrineとして局所で作用し,侵襲に対する生理的反応に不可欠な物質である。しかし,高度の侵襲によりサイトカインが多量に産生されたり,サイトカイン産生の制御機構が破綻すると,血中に種々のサイトカインが高濃度検出されるようになる。このような高サイトカイン血症で惹起される過大な全身性炎症反応から,自己破壊的なショックや臓器障害が起こる。この状態を筆者はサイトカイン・ストーム(cytokine storm)と称している。種々の病態におけるサイトカインの役割の解明とともに,抗サイトカイン療法が新しい治療法として注目され,とくにキー・メディエータであるTNFやIL-1の制御を目的として,抗TNF抗体,IL-1ra, TNFsrの応用が試みられている。抗サイトカイン療法はリスクの高いサイトカイン・ストーム下の患者でその効果が期待される。" @default.
