- W2062305940 abstract "One of the exercises I do with the residents is the “nightmare scenario” or “what is the worst fear you have in practicing emergency medicine?” Everyone has one’s own specific fear; the footling breech in labor has always been mine. Recently, however, a general fear has been pervading emergency physicians, weapons of mass destruction (WMD), formerly known as nuclear, biological, and chemical (NBC) agents but now including explosive, incendiary devices (BNICE). The Oklahoma City and World Trade Center explosions, the Tokyo subway sarin gas exposure, and similar incidents worldwide have engendered these fears. For 40 years, the fear of thermonuclear war was the basis of massive defense spending and civilian training throughout the world. Although the risk of thermonuclear war probably has not diminished, the threat of terrorist activities has become a reality that requires similar spending and training. This work grew out of the need to identify priorities for research and development in the areas of biological and chemical terrorism. The driving force behind this was the Federal Emergency Management Agency and the Department of Health and Human Services Office of Emergency Preparedness, the agencies that will undoubtedly be at the forefront of any terrorist incident in the United States." @default.
