- W2062777825 abstract "Physical therapists perceive a growing harshness in what has come to be known as managed health care in the United States. They are concerned that the momentous health policy initiatives of the 1980s and the apparent increasing restrictions of the 1990s will negatively affect their ability to deliver what they believe to be optimal care for their patients. Central to these concerns is the knowledge that physical therapists have not been directly involved in formulating health policy. Unfortunately, the physical therapist's ability to practice frequently depends on the regulatory, reimbursement and competitive milieus in which she or he practices. Traditionally, the emphasis in physical therapy has been on assessment and treatment with little emphasis on what is or is not effective. A good example of this would be the treatment of low back pain. Consider the predicament of a 28 year old female nurse who considered herself to be in excellent health until she suddenly developed low back pain. In the ensuing three weeks, the back pain went away without any medical attention. However, six months later a second, more painful bout of back pain and leg pain developed with weakness of the triceps surae. Bed rest and medications provided little relief. If the patient chooses nonsurgical treatment, what does the physical therapist need to know to encourage/maximize the healing process? First, the physical therapist needs to know the probable outcome of one treatment versus another. Where does this information come from? Often such knowledge is derived from experience supplemented by what one gleans from the published literature. However, the literature does not offer much in the way of help. In order to choose the most effective and efficient treatment for this patient and many like her and prosper in an era of limits, we need to have a greater understanding of what is or is not effective. We must evaluate our outcomes critically and publish our findings. Only by doing this will we be able to set the agenda." @default.
