- W2063313192 abstract "We welcome the comments of Rosendaal et al on the data analysis in our paper (Arya et al, 2002). We used this case–control design to minimize referral bias, but accept that analysis of those with and without travel history, stratified for the presence or absence of risk factors would be an appropriate comparison to determine the relative contributions of these factors. The relevant data from our study was analysed and is presented (Table I). Nine per cent of subjects presenting with suspected deep vein thrombosis (DVT) to our DVT clinic in Camberwell had travelled for > 3 h in the preceding 4 weeks. Travellers with risk factors had a significant increase in the risk of DVT with an odds ratio of 2·9 [95% confidence interval (CI) 1·1–7·9] when compared with non-travellers with risk factors. No such association was seen in those without risk factors. This more detailed analysis supports our findings of a relationship between travel and DVT in those with risk factors. We also examined the overall relationship of the predefined risk factors and DVT, and confirmed a significant association (Table I). Our recommendation to consider chemical as well as other thromboprophylactic measures was based on empirical data and current practice. Both analyses of our data indicated an increased risk of DVT, and the identification of this subgroup enables targeting of such empirical therapies. In our paper, we recognized the possibility of public and media interest biasing the referral pattern. With regard to the comment from Rosendaal et al that 8% of control subjects giving a travel history may be an unduly high figure for our population, our experience suggests this is likely to be a valid figure as Camberwell is an inner city suburb in south London and is an ethnically diverse community. Thirty-four per cent of the local population originate from Africa, the Caribbean and South Asia, and not infrequently travel to their country of origin on holiday. Inner city dwellers often travel out on weekends and, furthermore, travel of any kind seems to be becoming more frequent." @default.
