- W2063822011 abstract "HISTORY: An 18 year-old high school defensive back was found supine with a flexed knee and an obvious valgus deformity of the left thigh after making a solo tackle of an opposing running back near the sideline. The player had no known illnesses or bone abnormalities. PHYSICAL EXAMINATION: There was point tenderness over the thigh deformity and diffuse tenderness over the distal two thirds of the thigh. Pulses, sensation, and motor function were intact, and vital signs were stable before and after slowly straightening the knee with gentle traction. The leg was splinted, an IV line was placed, IV narcotic analgesics were administered, and the patient was transported to a local university hospital. DIFFERENTIAL DIAGNOSIS: Diaphyseal femur fracture Distal epiphyseal femur fracture Internal derangement of the left knee Hip/pelvic fracture Femoral artery compromise Popliteal artery compromise TEST AND RESULTS: Left femur anterior-posterior and lateral radiographs: Transverse overriding fracture of the midshaft femur FINAL DIAGNOSIS: Left transverse diaphyseal femur fracture TREATMENT AND OUTCOMES: 1. Traction pin placed in distal femur, with leg in traction for approximately 12 hours. 2. A left femur intramedullary nail with locking screw was surgically placed. 3. Due to tension and swelling in the left upper leg, a left thigh lateral compartment fasciotomy was performed to prevent impending compartment syndrome. 4. Discharged two days after surgery with toe-touch weight bearing of the left leg. 5. Two weeks after surgery, the fasciotomy wound was debrided, irrigated and closed without complications. 6. Four weeks after surgery the patient remained non-weightbearing and started range of motion physical therapy. 7. He is expected to make a full and complete recovery with a return to full activity and competition." @default.
