- W2063826522 abstract "Introduction: Home parenteral Nutrition (HPN) has given life to patients with intestinal failure who would otherwise have died. Nevertheless, morbidity from this therapy remains substantial although inferior to the rate in the inpatient setting. Methods: We report our experience on HPN in infants and children during the period 1993–2003. Complications were reported as rate per 1000 days of HPN and compared to the rate as inpatients. Results: 15 children (6 boys, 9 girls) aged 5 months to 7 years (median 13 months) recieved HPN for a median length of 150 days (range, 30 to 690 days). 11 patients suffered from chronic intestinal failure (3 short bowel syndrome, 3 severe motility disorder, 4 intractable diarrhea of infancy and 1 gastroschisis) and another 4, severe malnutrition that did not respond to enteral support. Only 17 central venous catheters were used (1.1 catheter per patient), 15 were tunnelled catheters and 2 subcutaneous ports. Catheter-related infections were the most usual complications. The relative risk for infection in the inpatient setting was 4.9 (CI 95%, 1.9–12.4) Only 2 childen presented severe hepatic involvement, one of the currently awaiting for a small bowell-liver transplant. Outcomes: 9 patients were weaned off HPN within the first year of therapy; 2 patients died, one because of the underlying disease (Langerhans hystiocitosis), one because of lack of venous access; 2 patients continue on HPN; 1 patient is awaiting for transplantation and 1 patient moved to other country.Table 1: Complications in HPN patients.Conclusion: The use of HPN is still scarce in our area (1.5 new patient/year). Most of the patients are able to be weaned off HPN within the first year of therapy. Although the rate of complications is still considerable, the relative risk is near 5 times higher if the patients remained hospitalized." @default.
