- W2065491881 abstract "Small intestinal transplantation is now a n accepted therapy for permanent intestinal failure. However, in spite o f intensive immunosuppressive regimens, rejection remains a significant cause o f morbidity and mortality. Therefore frequent endoscopic and histologic screening o f patients is imperative. Although operational definitions have been established for histologle rejection, little is known about anatomic variations with regard to the presence and absence o f rejection. Thus we conducted a retrospective analysis o f our endoscopic and histologle data 'on all pediatric patients who underwent intestinal or multivisceral transplantation at Children's Hospital o f Pittsburgh between July 1990 and May 1994. Patients and methods: Thlrty-two of 34 transplanted patients survived to undergo endoscopy. They were evaluated during a total o f 731 endoscopic sessions comprising upper endoscopy, ileoscopy, or coinnoscopy. On 188 occasions in 29 patients, biopsies were obtained from two or more anatomic regions (proximal and distal small bowel or small bowel and colon). I-Iistologic rejection was defined as epithelial apoptosis, eryptitis, and lamina propria infiltration with activated lymphocytes and/or ensinophils. Results: On 141 occasions, neither site showed rejection. On 27 occasions, rejection was present in both sites. Ten times rejection was identified proximally but not distally, and 10 other times rejection was present distally but not proximally. Conclusions: We conclude tha t anatomic variability for rejection may exist. When rejection is suspected clinically, biopsy from a single site may be inadequate.
