- W2065576551 abstract "Pregnancy is a physiologic state associated with substantial and, on occasion, profound alterations in metabolic and biochemical processes. If there are no preexisting diseases or complications, such functional changes are reversible following delivery. Many of these changes are reflected in altered concentrations of chemical and hematologic constituents of blood. Most pregnancies progress with no maternal or fetal complications and result in delivery of healthy infants. Problem pregnancies, once difficult to detect, are now identified by an assessment of appropriate environmental and genetic factors and by the judicious use of laboratory tests in the antenatal period. The recent introduction of new biotechnologies and of high-resolution imaging systems has fostered the establishment of comprehensive clinical programs for prenatal diagnosis. In spite of these advances, certain maternal diseases continue to constitute threats to the fetus and mother. Intercurrent diseases of pregnancy can be divided into two categories: diseases occurring in all population groups including pregnant women (e.g. essential hypertension, hepatitis, pancreatitis, urinary tract infection, etc.), and diseases unique to pregnancy. In this critical review we discuss the first group only briefly, since there are no significant differences in clinical presentation, differential diagnosis and management of these diseases between pregnant and non-pregnant women. The primary focus of this paper is a review of pathophysiologic mechanisms of certain syndromes that are unique to pregnancy, e.g. preeclampsia, eclampsia and fatty liver of pregnancy, and an assessment of the utility of diagnostic tests with particular emphasis on tests of liver dysfunction and the coagulation system.
