In the 1970s and 1980s, delayed cord clamping and stripping the cord were associated with polycythemia and resulted in frequent partial exchange transfusions. For whatever reason, polycythemia has become infrequent and the daily partial exchange transfusions have virtually disappeared. A frequent clinical response to the preterm or sick term infant has been to give a volume infusion for low blood pressure or perceived hypervolemia—fortunately a practice that is becoming less frequent. Humans evolved to survive with deliveries of infants below the uterus and without rapid cord clamping, so perhaps the placental to newborn “transfusion” at delivery is desirable. Recent trials and clinical experiences in both term and preterm infants document benefits of delayed cord clamping with minimal risk of polycythemia. In this issue of The Journal, Takami et al report their experiences with “cord milking” for deliveries of very low birth weight infants using ultrasound to evaluate cardiovascular performance and near-infrared spectroscopy to evaluate brain oxygenation sequentially to 72 hours after birth. Hematocrit increased and multiple measures of cardiovascular performance and brain oxygenation improved with milking the cord, a procedure that takes just a few seconds. Delayed cord clamping, or this milking procedure, probably does not increase blood volumes to high levels, but rather increases blood volumes to normal levels, which is an advantage for the newborn. Such procedures should be particularly helpful for infants delivered by cesarean birth.
