• W2067081687 abstract "Pancreatic cysts (PC) are diagnosed with increasing frequency while the differential diagnosis of cystic pancreatic lesions remains difficult. Endoscopic ultrasonography (EUS) is regarded a valuable technique. Aim of this study was to assess the degree of interobserver agreement of EUS in the diagnostic work up of PC in three different groups of observers with different levels of experience with EUS. Representative video sequences of 40 EUS procedures of PC including IPMNs, mucinous cystic neoplasms, serous cystic neoplasms, cystic neuroendocrine tumors and pseudocysts were selected. In 22 of the 40 cases histopathologic confirmation was available. Three different groups of observers were formed with different levels of experience in EUS. Group 1 consisted of 4 experts with extensive experience in EUS (> 2500), group 2 of 4 semi-experts with experience in EUS (50-200) and group 3 of 4 non-expert residents without experience in EUS. Before reviewing the video sequences all observers were exposed to a training set demonstrating specific EUS-features of PC. Observers were blinded to clinical and histological results. Features scored included: septations, nodules, solid components and pancreatic duct communication. Furthermore, a presumptive diagnosis and the suspicion of malignancy had to be specified. Intraclass correlation coefficient (ICC) was used to measure agreement within the groups. ICC values greater than 0.81 represented excellent, 0.61-0.80 good, 0.41-0.60 moderate, 0.21-0.40 fair and <0.20 poor agreement respectively.
