We recently encountered ASLBV in an adult organ donor with normal cardiac anatomy on echocardiogram. Further evaluation showed that the left brachiocephalic vein was missing from its usual location and that the left superior vena cava (SVC) was absent. The ASLBV took an unusual retroaortic course as it crossed left lateral and beneath the arch, above the pulmonary artery, and behind the ascending aorta to join the SVC near the azygos vein (Fig 1A, arrow). Before donor cardiectomy was performed, the ASLBV was ligated and divided away from the SVC to avoid its narrowing. The technique for implantation in the recipient was unchanged. Although the thoracic central veins could not be evaluated in the absence of computed tomography of the donor's chest, roentgenology of the chest showed an unusual course of the left central line catheter (Fig 1B, arrow).Although ASLBV is rare and in itself is asymptomatic, recognition is important to avoid misinterpretation [2Nakamura R. Difficulty may be encountered during insertion of a central venous catheter through the left jugular or subclavian approach. Moreover, the lower insertion of the ASLBV into the SVC requires careful evaluation to choose an appropriate, more caudal SVC cannulation site.
