- W2068334105 abstract "Introduction: Blood testing using hematocrit (Hct) or hemoglobin (Hb), platelet count (Plats), albumin (Alb) and sedimentation rate (ESR) are commonly used in evaluating IBD patients. The aim of this study was to correlate blood tests with disease characteristics at diagnosis in children with IBD. Methods: Children with newly diagnosed IBD, prospectively enrolled in an observational registry at 18 US/Canadian centers are included in analysis. Results: 333 children[mean age 8.2 years; 59% males; 250 Crohn’s disease (CD), 83 ulcerative colitis (UC)] had all pertinent lab tests at diagnosis. Among the CD patients, 8% (n=20) had all lab tests normal at diagnosis. Among those with moderate or severe CD based on the Physician’s Global Assessment index, 8 of 173 (4.6%) were found to have all labs normal. For individual lab tests, a normal Hct, Plats or Alb was found in 50%, 43% and 45% of moderate/severe CD patients, respectively, but only 14% of these patients had a normal ESR. CD distribution did not discriminate those patients with normal from abnormal blood tests with equal numbers of patients having large bowel only, small intestinal only, both large and small bowel or gastroduodenal involvement. Pancolitis was present in 75/83 (90%) UC patients at diagnosis. The severity of UC was moderate/severe based on the Physician Global Assessment index in 60 (80%) of the pancolitis patients. All labs were normal in 10% of patients with moderate/severe UC. Normal Hb, Plats, Alb or ESR was seen in 30%, 62%, 57% and 44%, respectively, for the moderate/severe UC patients. UC patients with pancolitis were more likely to have abnormal lab tests as compared to those that did not have total colonic involvement. Conclusion: Standard lab tests are normal in a small percentage of newly diagnosed children with CD or UC even in the presence of moderate to severe disease activity. The abscence of abnormal laboratory screening should not preclude further evaluation in a child who presents with signs and symptoms of inflammatory bowel disease." @default.
