- W2068593399 abstract "Epidural analgesia, a commonplace and long-standing technique, has been recently refined by advances in our knowledge of epidural anatomy, spinal cord pharmacology, newer methods of administration, and combination of spinal and epidural techniques. Currently the emphasis is to provide excellent analgesia, but not complete anaesthesia, while minimizing the degree of motor block and allowing women to ambulate safely during labour. Whether this emphasis will result in improved obstetric outcome is debatable, but may well result in an improved sense of patient well-being. In order to achieve this analgesia with minimal motor block, epidurally administered local anaesthetics must be carefully titrated, either by anaesthesia care givers or by the patient herself, and must be combined with other agents, most commonly opioids. Initiation of labour analgesia with a spinal injection of opioid alone or with a very small dose of local anaesthetic allows for rapid onset of excellent analgesia of prolonged duration with minimal motor block. Spinal administration may also allow for use of opioids and other receptor-selective agents alone with minimal systemic side effects." @default.
