- W2068704843 abstract "Purpose: Secondary membranes are a common problem following pediatric cataract extraction. In many cases it is of sufficient density to cause obscuration of the visual axis and requires removal. While in adults this is usually accomplished as an in-office laser procedure, most pediatric patients must return to the operating room for surgical removal. This has been accomplished with intraoperative YAG laser, a pars plana/posterior approach, or a limbal approach. Method: We performed 25 secondary membranectomies using a limbal approach. This was accomplished by entering the anterior chamber and gaining access to the membrane by making a small iridectomy and passing the vitrector through the opening to go behind the lens or by tilting the IOL to manipulate the vitrector behind the lens. The membrane was then removed and an anterior vitrectomy was performed. Results: Secondary membranectomy was successfully accomplished in all patients; one required an additional procedure. No lenses were dislocated while performing the surgery. Postoperative inflammation and discomfort were minimal in all patients. Conclusions: A limbal approach is an effective method for performing secondary membranectomy with few complications. In pediatric patients with small eyes, this avoids a more technically difficult pars plana approach." @default.
