From a review of the works published by Brock, 1 Zdansky, 2 Graham and co-workers, 3 Paulson and Shaw, 4 and others, 5 there seems to be agreement that it is due in most cases to bronchial obstruction caused either by pressure from without or by narrowing of the lumen due to formation of chronic inflammatory tissue within. The commonest cause is pressure from without, usually due to enlargement of the peribronchial lymph nodes. As emphasized by Brock, the middle lobe bronchus lies at the apex of the lymphatic pathways from both the upper and lower lobes thus making it particularly vulnerable from the standpoint of lymphadenopathy.
