- W2070011360 abstract "Le mal de Pott de la charnière cervico-occipitale (CCO) est exceptionnel. Il expose à de graves complications bulbomédullaires. Les auteurs rapportent trois cas de tuberculose de la CCO à partir d’une série de 63 cas de tuberculose vertébrale. L’âge moyen des patientes est de 51 ans (extrêmes : 20–69). Le délai moyen de diagnostic est de 4,6 mois (extrêmes : 1–8). Dans les trois cas il n’existait pas de déficit neurologique malgré l’existence d’une luxation atloïdo-axoïdienne. Le scanner cervico-occipital a orienté le diagnostic en montrant des images lytiques de la charnière cervico-occipitale et un abcès prévertébral dans les trois cas. Le diagnostic fut bactériologique dans deux cas et histologique dans un cas. L’évolution fut favorable dans les trois cas sous antibacillaires, immobilisation cervicale et arthrodèse chirurgicale. Malgré la fréquence de la tuberculose ostéoarticulaire dans les pays en voie de développement, le mal de Pott sous-occipital est rare. Il peut exposer à une grande instabilité et à de graves complications bulbomédullaires. Il nécessite un diagnostic et un traitement rapides.Atlantoaxial tuberculosis is uncommon, produces atypical clinical manifestations, and can cause serious compression of the spinal cord and medulla oblongata. Three cases seen over a 14-year period among 63 patients with tuberculous spondylitis were reviewed. Mean age was 51 years (range, 20-69). Mean time to diagnosis was 4.6 months (range, 1-8). Neck pain and torticollis were consistent features. Although all three patients had atlantoaxial instability, none had neurological deficits. Computed tomography showed lytic lesions of the craniocervical junction and an abscess anterior to the spine. The diagnosis was confirmed by microbiological studies in two patients and histological studies in one. The outcome was favorable after antituberculous drug therapy, immobilization of the neck, and surgical arthrodesis. Atlantoaxial tuberculosis is rare, even in developing countries where tuberculosis remains endemic. Because major instability and serious complications due to compression of the spinal cord and medulla oblongata can occur, early diagnosis and treatment are of the utmost importance." @default.
