The differences between emetic effects of cytotoxic drugs can be related to whether they act on DNA, RNA, or protein synthesis. Enzymes that degrade enkephalins are a potential target. Most anti-emetics are dopamine antagonists and are only marginally effective. Opiates are emetic and many of their effects are mediated via dopamine systems. Opiates also have anti-emetic effects that may be mediated via a medullary anti-emetic centre. Inhibition of enkephalin production in this centre may potentiate effects at the area postrema. Opiates and selective antagonists should be assessed for anti-emetic activity." @default.
