- W2070911501 abstract "The promise of new disease-modifying therapeutics has created a great need for sensitive and reliable diagnostic tools for AD. The cerebrospinal fluid (CSF) biomarkers total tau (T-tau), reflecting neuronal degeneration, Aß42, reflecting the deposition of A? in plaques, and phosphorylated tau (P-tau), probably reflecting tangle formation, have consistently been found to have high diagnostic accuracy for AD. Recent large multi-center studies have also confirmed that these biomarkers can identify cases with prodromal AD in MCI cohorts. CSF biomarkers show very low biological variability in individual cases. Repeated analysis of internal control samples (aliquots of pooled CSF) also show stable levels in individual laboratories. However, these CSF biomarkers have a relatively high between-lab variability, which will hinder a more general introduction in clinical routine, and also make their use in multi-center clinical trials complicated. Poor between-lab variability may be due to pre-analytical factors (e.g. differences in CSF tapping and sample handling), analytical factors (differences in how the analyses are performed) and also to batch-to-batch variation for the immunoassays. For this reason, we have, together with The Alzheimer's Association initiated a world-wide quality control (QC) program for CSF biomarkers, with the objective to standardize CSF biomarker measurements between Labs. The program consists of two parts, a standardized protocol for lumbar puncture (LP) and CSF sample processing, and an external QC program for comparison of CSF biomarkers between Labs. The QC program will allow a comparison of biomarker levels between Labs, and will also allow evaluation of the longitudinal stability for CSF biomarker levels. The QC program is open for the generally (commercially) available assay formats that are used on a routine (clinical or research) basis. Today, 40 laboratories are participating in the QC program.
