- W2070923714 abstract "Brace prescription after ACL reconstruction (ACLR) is controversial (McDevitt 2004, Sterett 2006), yet most US ACL surgeons recommend brace use for return to cutting and pivoting sports (Decoster 2003, Delay 2004). All active patients in our practice are braced and must pass a sport battery of tests before they return to play (RTP). Criteria include 90% score on 4 one-legged hop tests (Noyes 1991), burst superimposition strength test (Snyder-Mackler 1995), Knee Outcome Survey ADL Scale (KOS-ADLS) and a global rating of knee function (Irrgang 1998). PURPOSE: To determine if athletes who undergo an ACLR can discontinue functional brace use one year after surgery. METHODS: Sixty-four patients were tested one year after ACLR using the same test battery with and without their braces. There were 29 women and 35 men with a mean age of 25 years. RESULTS: Mean KOS-ADLS score was 98%, global rating was 97% and KOS-SAS (Sports Activity Scale) score was 94%. In general, subjects performed better unbraced than braced in all hop tests: single leg hop braced = 101%; unbraced =107% (p=0.000); cross-over hop braced=100%; unbraced=105% (p=0.000); triple hop braced = 99%; unbraced =101% (p=0.003); timed hop braced = 98%; unbraced = 103% (p=0.004). Six of the 64 patients failed one or both tests. One patient failed hop testing both with and without the brace. Four patients failed the test while braced but passed unbraced, while one additional patient passed with the brace, but failed without the brace. CONCLUSIONS: Sixty two of 64 patients continued to score above RTP criteria one year after ACLR in at least one condition. All patients but two, in the cohort performed better unbraced than braced. Criterion based functional testing resulted in successful RTP in 62/64 individuals, and the recommendation to discontinue brace use one year after ACLR." @default.
