J Pediatr 1957;50:226-30 In 1956, Yanet classified the underlying causes of mental retardation for more than 2500 admissions to the Southbury Training School for Mentally Retarded in Connecticut. Only 40% of these children were mildly retarded, as compared with the 75% prevalence of mild mental retardation in the larger community; this reflected the selection bias of which children were institutionalized. Ninety-one percent of the causes were categorized as prenatal, 3% perinatal, and 6% postnatal. The prenatal causes were divided into those that were definitely genetic (only 5% had recognized single-gene disorders) and those that were “not definitely genetically determined.” Oddly, “mongolism” (Down syndrome) was classified in the latter group; the influence of maternal age was thought to reflect some yet unidentified environmental influence, and Jerome Lejeune would not report the chromosomal duplication for trisomy 21 until 1958. A long-standing miscount in the total number of human chromosomes had contributed to the delay in making this identification. Down syndrome nevertheless accounted for 80% of the 10% of the prenatal group that included prenatal infections, kernicterus, and cretinism. Of the other 81% with prenatal causes, almost half (40%) represented “familial” or “subcultural” retardation; there were both environmental deprivation as well as “physiological” contributions in the form of the derivative genetic pool for this group. A large number of children were placed in the genetic group with minimal specific findings (eg, isolated microcephaly), so that many of those would today probably be classified as idiopathic. The 3% incidence for perinatal cause seems low when considering the history of malpractice lawsuits over the following half century. Although fetal alcohol syndrome, fragile X syndrome, and many other specific causes were still in the future, the shape of the distribution of causes has nevertheless remained remarkably constant." @default.
