- W2071503204 abstract "Aims: To evaluate endoscopy sedation provided by nurses under supervision of endoscopist. Patients & Methods: Consecutive patients with ASA grade I-III undergoing endoscopy were enrolled from Jan 04-Oct 04 after informed consent. Sedation was induced with midazolam, pethidine was added in anticipation or presence of pain & propofol was added further if any discomfort/unrest noticed with dosages titrated as required. All patients received 2 L/min oxygen and were monitored with pulse oximetry & automated sphygmomanometry & an anaesthetist's services were kept standby. The patient's comfort and satisfaction were assessed after recovery & over phone at 24 h. Results: In total 583 procedures, Patients' Age Mean ±SD (range): 45.34 ± 14.90(6-91)y, M/F: 1.06/1, 83.5 % (487) were ASA grade I –II & 16.5 %(96) III. The medication used for sedation was: Average dosages (range) in mg were: Midazolam 3.8 (1-11), Pethidine 36.3 (12.5-100) & Propofol 104.4 (20-340). No patient had significant oxygen desaturation (<85%) when midazolam alone or with pethidine was used, while it was observed in 21.2 % (18) when propofol was added. Hypoxemia was transient in 77.8% (14) patients while 22.2% (4) had apnoeic episodes. Three such patients improved by physical stroking while the fourth who had rolling back of the tongue, recovered by applying oropharyngeal air way. Mean BP temporarily fell < 50 mmHg in 22.4 % (19) patients & 2 (10.5 %) of them needed saline infusion. None of the procedures needed anaesthetic services or was terminated for lack of inadequate sedation. All patients denied unpleasant recall and were satisfied. Conclusions: Safe & efficient endoscopy sedation can be administered by nurses under supervision of endoscopist. Midazolam alone suffice for majority of UGIs while most colonoscopies need addition of pethidine. Propofol augments control further which is required for remainder of them & nearly all ERCPs with a manageable risk of oversedation." @default.
