- W2071737215 abstract "Frontotemporal dementia (FTD) is a neurological disorder characterised by the progressive degeneration of the frontal and anterior temporal cortices. According to the FTD criteria (Neary et al 1998), FTD is a behavioural syndrome characterized by changes in personal and interpersonal conduct. Onset of the disorder is insidious and gradually progressive with the first symptoms occurring most commonly but not exclusively before the age of 65. However, a definite diagnosis can only be established after postmortem pathological examination. When the clinical aspect is atypical, cerebrospinal fluid (CSF) biomarkers could be useful to eliminate the diagnosis of other inflammatory, infectious diseases or Alzheimer's disease (AD). We report a case of an 80-year-old woman who was addressed to our memory disorder clinic as a result of behavioural disturbances and personality changes. We performed several neuropsychological tests, MRI imaging and CSF analysis. Neuropsychological tests revealed cognitive deficit in attention, abstraction, planning, problem solving and some troubles in verbal memory. The patient's memory complains was considered as secondary to the dysexecutive syndrome. The patients’ orientation and instrumental functions were intact. All behavioural and clinical characteristics were corresponding to FTD criteria. After several months, we noted clinical and neuropsychological worsening associated with verbal memory impairment. A brain MRI revealed significant atrophies localized in both anterior temporal lobes while FDG PET scan showed posterior hypofixation atypical for AD. The CSF of tau-protein (600 pg/ml), phosphorylated tau (87 pg/ml) were increased, whereas the concentration of beta-amyloid protein (Abeta1-42) was decreased (480 pg/ml) according to our cut-off (Dumurgier et al.) suggesting AD CSF profile. Among different clinical AD phenotype, FTD clinical presentation must not be misdiagnosed. CSF biomarkers are helpful to diagnose these patients, establish an accurate diagnosis and introduce a specific treatment." @default.
