- W2071738772 abstract "Zusammenfassung: Wir berichten über einen außergewöhnlichen Chromomykosefall, bei dem neben dem seltenen Erreger Fonsecaea compacta zum ersten Mal am Menschen Phaeosclera dematioides isoliert wurde. Definitionsgemäß handelt es sich demnach urn eine Chromomykose, bei der durch den Schwärzepilz Phaeosclera, der üblicherweise an verholzten Pflanzen auftritt, sekundär eine “phaeohyphomykotische Besiedlung” auftrat. Als klinische Läsion imponierte eine den gesamten Fußrücken überziehende verrucöse Platte, die fan Verlauf von 2–3 Jahrzehnten entstanden war. Histologisch lag die typische pseudoepitheliomatöse Hyperplasie vor; an Pilzelementen wurden fan Corium wenige einzeln liegende dunkle Sclerotialzellen und einige in Gruppen liegende Rundzellen fast ohne Pigmentierung gefunden, nirgends jedoch Hyphen. Zur Therapie wurde Ketoconazol verordnet – 400 mg/Tag –, da befan in-vitro-Test beide isolierten Pilze auf diese Substanz am besten ansprachen. Summary: We report on an exceptional chromomycosis whereby in addition to the rare fungus Fonsecaea compacta, Phaeosclera dematioides could be isolated for the first time in a human being. This process can be defined as a case of chromomycosis, where a secondary “phaeohyphomycotic infection” with the fungus Phaeosclera (normally found on ligneous plants) took place. Clinically, a verrucous plate covering the entire back of the foot was observed which had grown to this size over a period of twenty to thirty years. Histologically, a typical pseudoepitheliomateous hyperplasia was seen. Fungal elements were found both in the form of a few dark sclerotial cells, as well as almost pigment-free round cells. Hyphae could not be found anywhere. The therapy consisted of Ketoconazol (400 mg/day), since both fungi, Fonsecaea and Phaeosclera, were found to be in vitro sensitive to it." @default.
