- W2072304016 abstract "The correct diagnosis in all patients in this group (Table I) was tuberculous lymphadenitis. In the first two patients the classical clinical features of tuberculous lymphadenitis were found, and in both a correct pre-operative diagnosis was made. The first patient, a child aged 3 years, was admitted with enlargement of the cervical lymph nodes associated with chronic tonsillar infection. Her tonsils were removed, and histological examination of them revealed tuberculous infection. Five months later enlarged submandibular and cervical lymph nodes were removed and these also showed extensive tuberculous involvement. The second patient, a woman of 22 years of age, had had tuberculous cervical lymph nodes removed at i I years of age, and she returned because of recurrence of the infection. Of the remaining seven patients in this group those with involvement of the parotid lymph nodes are of particular interest, because here a correct diagnosis was never made pre-operatively. These patients came complaining of a swelling in the parotid region which they often described as recently increasing in size. Usually the swelling had been t~esent for two or three months, but in one patient there was a 20 year history. Pain was not an outstanding feature, but some patients reported occasional pain. The swellings measured approximately 2 in. by i1⁄2 in., were firm, and had a smooth surface. The degree of fixation varied, but facial nerve function was always normal. Even at operation these lesions were usually considered to be tumourous. In the three older patients with submandibular swelling, numbers 5, 6 and 7 in the table, the most common provisional diagnosis was chronic sialadenitis associated with duct obstruction. Again the history in these patients was short and, although increase in size with eating never occurred, all the swellings were occasionally painful. In all the cases in this group the diagnosis of tuberculous lymphadenitis was made on the finding of granulomatous inflammation associated with caseation in the biopsy material. Tubercle bacilli were demonstrated in four of the nine cases by direct examination of the biopsy material, but cultural techniques and animal inoculation were not routinely employed, since the diagnosis was so rarely suspected at operation.
