We have already reported that double-balloon endoscopy with an overtube enables endoscopic access to the entire small intestine. In this study, we evaluated the usefulness of double-balloon endoscopy in the afferent limb including pancreatobiliary disorders in patients with Roux-en-Y operation. Methods: We had 8 consecutive patients with Roux-en-Y reconstruction that have undergone trans-oral double-balloon endoscopy for the examination of the afferent limb between April 2002 and November 2004 at Jichi medical school hospital in Japan. In addition to the endoscopic observation, X-ray imaging examinations with contrast and/or endoscopic treatments were carried out whenever necessary. Results: Six patients underwent the procedure for endoscopic retrograde cholangiopancreatography (ERCP). Double-balloon endoscopy could reach the deepest part of the afferent limb in 5 of 6 patients (83.3%) and the endoscopic examinations were successful without complications in all these 5 patients. Selective endoscopic retrograde pancreatography and cholangiography were performed in 1 and 4 patients, respectively. They included endoscopic biliary stenting (n = 2), and biliary balloon dilation (n = 1). The remaining 2 patients underwent the procedure for the observation of the afferent limb lesions. Biopsy (n = 1) and endoscopic mucosal resection (n = 1) in the terminal part of the afferent loop were successfully performed. Conclusions: Double-balloon endosopy is applicable for diagnosis and treatment of lesions in the afferent limb of Roux-en-Y operation." @default.
