We investigated the potential relationship between anxiety and lower urinary tract symptoms (LUTS) in a cohort of men enrolled in AS. Methods A prospective cohort study was conducted of men on AS. Men underwent repeat (confirmatory) biopsy within 6 months of enrollment. All men completed the Memorial Anxiety Score for PC (MAX-PC) and AUA Symptom Index (AUA-SI) questionnaires at enrollment and repeat biopsy. Comparisons were made of MAX-PC and AUA-SI scores based on time until repeat biopsy (<3 months versus >3 months). Some analyses involved comparisons with the AUA-SI irritative (Q2,4,7) and obstructive (Q1,3,5,6) subdomains. Regression analysis was used to examine the impact of age and anxiety on LUTS. Results 195 men (mean age 66.5 years) met inclusion criteria. Mean prostate volume was 47g, and mean PSA was 5.2ng/ml. When comparing the group of men with a scheduled prostate biopsy within 3 months versus those with a more distant biopsy, the MAX-PC total score and two of its subscores (prostate cancer anxiety and PSA anxiety) were found to be statistically higher (p=0.03, p=0.007, and p=0.06, respectively), indicating an increased level of anxiety in the <3 month group. On multiple regression analysis, both age (p=0.045) and MAX-PC total score (p=0.064) were predictive of higher AUA-SI scores, accounting for 11% of its variability (r2=11.4%). When performed on the AUA-SI subcategories, age and MAX-PC total score were predictive of irritative symptoms (p=0.010 and p=0.032, respectively), but not obstructive symptoms and accounted for 15% of irritative score variability (r2=15.3%). Conclusions The time from prostate biopsy was inversely related to anxiety level, but not LUTS. Anxiety, specifically the MAX-PC score, as well as age, were predictive of higher AUA-SI scores and irritative symptoms.
