- W2072667738 abstract "Obesity in childhood is a major health care problem. A variety of therapeutical concepts exists. The major drawback is the lack of long term control – that is long term maintenance of normal weight. We established a multi-professional obesity program in a large paediatric private praxis focusing on physical activity and dietary restrictions. A major aspect of this program was to establish a method to study self-perception and body image, as well as to evaluate self control in obese children and their families. The program consists of clinical examinations, nutrition teaching and sports lessons. From 01/2004 to 11/2005 161 children (82girls, 79 boys; median age 11.3 years (range 11mo to 19y); median BMI 24.6kg/m2 (range 18.1–41.9) were enrolled. In 129 children (80%) an extensive metabolic and hormonal baseline blood testing was done; cardiopulmonary exercise testing (CPET) was done in 80 children (49.7%) including treadmill exercise using a standard protocol (Bruce). In a large number of children detailed examinations revealed characteristic features like acanthosis nigricans, gynaecomastia and striae distensae. 74 of the 129 patients (57.4%) had abnormal biochemical values, and 66 of 80 (82.5%) patients had pathologic results in the CPET including a pathologic BP in 28.8%. Only 35 children participated in the nutrition teaching (21.7%) and just 13 children joined the sports program (0.8%) regularly. Follow-up weights were taken from 46 patients (28.6%), 13 children were able to loose and 13 were able to keep their weight; 20 increased their weight. Most obese children already demonstrated pathologic changes in clinical, laboratory and cardiovascular testing. Nutrition teaching and physical exercise were used by a small number of children demonstrating a lack of interest and motivation to change their lifestyle. In our experience long term weight reduction can only be achieved with frequent patient contacts. Local ambulatory centres should be established where overweight children can be looked after consistently and regularly over an extended period of time." @default.
