During his initial hospitalization, he experienced several complications, including abdominal compartment syndrome, pulmonary embolism, and acute respiratory distress syndrome, ultimately requiring approximately 3 months of mechanical ventilation. These injuries kept him completely immobile for many months. Four months after hospitalization, he was diagnosed with heterotopic ossification (HO) of the knees, shoulders, and pelvis (Figs 1-3). Heterotopic ossification is a process wherein bone is formed in normally noncalcified soft tissues around a joint. While the exact mechanism for this anomalous development of bone is unknown, multiple theories exist and include the following: (1) the transformation (perhaps by an unknown hormonal agent) of mesenchymal cells in the soft tissue into osteoblastic cells, which then produce lamellar bone; (2) formation of lamellar bone by the process of active remodeling with normally differentiated osteoblasts and osteoclasts that have migrated away from true bone tissue; and (3) fibrodysplasia ossificans progressiva, a rare inherited disease. In any case, the result of the heterotopic bone production can produce clinically relevant symptoms, including significantly decreased range of motion and pain about the affected joint.
