- W2073469651 abstract "Varon et al. discussed an unusual occurrence with an 81-year-old comatose survivor of cardiac arrest who was treated with therapeutic hypothermia using a surface cooling device. The author elucidated the patient's extensive medical history including cardiac, pulmonary and renal disease. Along with this medical history, the picture of the incident shown in this case study has provided some insight into the potential cause of this event. In the minimal evidence presented here, one would question whether the patient's cardiovascular status may have been too severely compromised to qualify her as an acceptable candidate for cooling. The fact that the patient was unable to rewarm at the end of the therapy bodes poorly for a favorable prognosis. Patients with an underlying history of congestive heart failure, end stage renal disease, interrupted dialysis, probable pressor and fluid support could potentially have caused edema and significant third-spacing with blistering resulting in blister rupture or de-roofing. This occurrence is not uncommon in terminal patients with traditional wound dressings as well. In our enthusiasm for this therapy that continues to provide remarkable outcomes, we must attempt to be vigilant about selecting the appropriate patient population to induce mild hypothermia regardless of the type of the technology or approach that is implemented. Inherently, the physician must ascertain the risk of any therapy versus the benefit for the patient. As we get more experience with cooling, hopefully we will be able to triage these patients and select the ones who will benefit the most, and exclude those who either will have minimal benefit or might actually incur harm, as it seems happened in the case presented.
