- W2073678664 abstract "As has often been the case in the history of the HIV pandemic, this is a time of unprecedented commitment. The focus now is on escalating not only HIV prevention, or only HIV care and treatment, but on delivering the whole package and for all who need it everywhere in the world. The debates are intense, as they have always been, on how the goals set for universal access can be achieved today, tomorrow and in the longer term. For some – call them the interventionists – skeptical about the capacity and willingness of states and populations to engage in structural and societal reform, these debates revolve around the nature, focus and priority that should be awarded to specific programme interventions. For others – the ideologists – change in human behaviors and practices cannot be effected beyond what has been demonstrated in localized projects, and success is primarily contingent on larger structural and societal reform. For most – the pragmatists – the optimal pathway to success calls for a combination of these approaches: success in prevention, care and treatment requires effective programming, ambitious target setting, inclusion and participation of HIV-infected and affected people, as well as non-discrimination and the equitable use of resources both within and across nations. In ongoing debates, the question is no longer whether universal access is a valuable goal, but rather how it can best be achieved: this is progress. The aim now is to move beyond interesting ideas and presentations at global HIV conferences to bring together in policy and in practice effective interventions that include efforts towards societal transformation, not only in rhetoric or as add-ons, but as central to successful action. The authors brought together in this supplement issue have individually and collectively devoted their attention to analysing what is usefully occurring in a variety of settings across the globe, considering where there are still gaps in knowledge, and determining what is necessary for expansion, improved outcomes and sustainability. In this issue, beyond the rhetoric of ‘enhancing political commitment’ or ‘reducing stigma and discrimination’, concrete steps are suggested to escalate the responses to HIV on the ground and within the environments in which they unfold. The theme of this issue centers attention on social justice and human rights as prerequisites for and critical outcomes of universal access. It is likely that no two contributors to this issue would define social justice or human rights in identical ways nor agree on the choice of concepts and values that provide the most appropriate framework for unpacking the complex linkages between actions on the ground and the required structural and societal changes. These frameworks, and others, including ethics, utilitarianism, equalitarianism, welfare and others, could equally serve to structure the diversity of issues discussed here. In fact they do, for none is irrelevant to this debate and all provide a meaningful contribution to the array of approaches suggested usefully to inform policies, programmes and action. What best characterizes this collection of articles is that beyond rhetoric and philosophical debates, the aim of all contributions is to suggest practical ways to move forward, in some cases through explicit recommendations, in others through directions for shaping or reshaping the environments in which the spread of HIV can be curbed and access to quality care and treatment made available to all who need it. The various articles presented here will by no means settle the debates as to what interventions are best prioritized or where resources are best spent to achieve universal access. Not all authors necessarily agree with the perspectives presented by others. This is healthy. A variety of perspectives cast on the epidemics and the responses they demand should continue to underpin our collective pursuit of better ways to achieve universal access. Even as the optimal choice of approach must be continuously debated and negotiated in light of new knowledge, capacities and experiences, the articles presented here offer critical food for thought and join the concerns of the interventionists and the ideologists with those of the pragmatists. It is hoped that this supplement issue will contribute to advancing our perspectives on achieving universal access and provide useful guidance to national and international actors, and most importantly to all who confront HIV in their daily lives.
