- W2075513726 abstract "Loss of independence in everyday functioning is part of the diagnostic criteria for dementia. Neuropsychological measures of memory and executive functioning have been associated with impaired instrumental activities of daily living (IADLs) in both cross-sectional and longitudinal studies of individuals with Alzheimer's disease (AD) and mild cognitive impairment (MCI). The purpose of this study was to examine the extent to which the FrSBe, an informant rating of frontal systems behavior, and neuropsychological test performance predicted functional decline in older adults. Participants (n = 124) were older adults with normal cognition, MCI and dementia who were referred for a clinical neuropsychological evaluation at a university medical center. Participants were administered neuropsychological tests of executive ability (Victoria Stroop, Category and Letter Fluency, Trail Making and the modified Wisconsin Card Sorting Test) and memory (Hopkins Verbal Learning Test-R, Brief Visuospatial Memory Test-R, Wechsler Memory Scale-III Logical Memory). An informant completed the FrSBe and the Functional Activities Questionnaire (FAQ). An executive composite (EC) and a Memory composite (MC) were calculated by assigning a 1 to T-scores in the normal range (> T36 or >7 th percentile), a 0 to T-scores in the impaired range (<T35) and summing the respective values for EC and MC. Bivariate correlations between predictor variables and FAQ were statistically significant (r = -0.43 EC; r = -0.30 MC; r = 0.59 FrSBe). A hierarchical regression analysis was used to examine the independent contributions from MC, EC and the FrSbe to IADLs. The overall model accounted for 47% of the variance in IADLs (F[3,120] = 36.2; p <0.001). At Step 1, the EC explained 19% of the variance, at Step 2 the MC did not account for additional variance, but at Step 3 the FrSBe explained an additional 28% of the variance. This study extended previous findings that both cognitive and behavioral measures of executive function contribute independently to instrumental activities of daily living in AD to a group of older adults with cognitive complaints, MCI and dementia. These results underscore the need for including informant ratings of executive function when evaluating older adults." @default.
