- W2076604111 abstract "Study objective: While ketamine blocks sodium and potassium currents in peripheral nerves, it is not clear if intra-articular ketamine offers peripheral analgesia. Design: This was a dose finding, open-label study examining the potential analgesic effect of ketamine administered intra-articularly following knee arthroscopy. Patients: 50 patients were assigned to one of five groups: saline, 0.6, 0.7, 0.8, and 1 mg kg−1 ketamine. Intervention: At the conclusion of surgery, the intra-articular dose of ketamine or saline (20 ml) was injected by the surgeon. Measurements: A patient-controlled analgesia morphine pump was used during the first hour in the PACU. Pain was measured using a 0–10 verbal numeric rating scale at 30 min, 1, 2 and 24 h after surgery. Results: Pain scores were lower (p = 0.04) at 30 min and 1 hour after surgery in the 1.0 mg kg−1 ketamine group compared with the saline control group. Cumulative morphine consumption for the one hour stay in the PACU was significantly lower in the 1.0 mg kg−1 ketamine group than in the saline control group. Conclusions: This study shows that intra-articular ketamine administered at a dose of 1 mg kg−1 at the conclusion of knee arthroscopic surgery results in a reduction in morphine consumption only in the first hour after surgery compared to placebo.
