- W2076646261 abstract "A 36-year-old woman without a surgical or medical history presented to the Emergency Department (ED) due to dull progressive lower abdominal pain for 4 days associated with leukorrhea. There was no nausea, vomiting, constipation, or diarrhea. The physical examination showed a body temperature of 37.6°C, a soft abdomen without distension, normal active bowel sounds, and tenderness over the lower abdomen with mild rebound. Laboratory studies revealed a white blood cell count of 16,900/mm3 with 66.9% segmented neutrophils. A pregnancy test was negative. A urinalysis showed no urinary tract infection. An abdominal computed tomography scan revealed hepatic portal venous gas (HPVG) (Figure 1, arrow) and mesenteric vein gas (Figure 2, white arrows), but there were no obvious gastrointestinal radiological abnormalities. Gynecology was consulted, and the patient was diagnosed with pelvic inflammatory disease (PID). She was admitted and received intravenous cefazolin, gentamycin, and metronidazole treatment. Her abdominal pain subsided gradually, and she was discharged 5 days after admission. Treatment was continued with oral antibiotics. There was no recurrent abdominal pain at the Outpatient Department follow-up 1 week after discharge.
