L’objectif de ce travail de recherche est de conduire une revue de la littérature scientifique sur le sujet de l’association entre troubles cognitifs et comportements violents dans la schizophrénie. Dans la schizophrénie, la littérature scientifique rapporte l’atteinte de diverses fonctions neurocognitives : attention, mémoire, fonctions exécutives et capacités de prise de décision. Chez les sujets « violents » ou déinis comme ayant des traits de personnalité psychopathique, il a été mis en évidence des troubles de l’attention, de la mémoire de travail et des capacités de prise de décision. Les sujets souffrant de schizophrénie, avec des antécédents de comportements violents, présenteraient des altérations cognitives caractéristiques (troubles de la reconnaissance faciale des émotions et des fonctions exécutives). Le développement de programmes de remédiation des fonctions cognitives altérées chez les sujets souffrant de schizophrénie permettrait, en outre, d’améliorer leur état de santé, mais aussi de diminuer l’occurrence de comportements violents. In healthy volunteers, without mental disorder, an association was shown between cognitive impairment and violent behaviour. It is now accepted that schizophrenia is associated with substantial alterations in cognitive function, and suggested that patients might exhibit violent behaviour. Thus, it seems relevant to test for an association between cognitive impairment and violent behaviour in schizophrenia. Articles relating evidence of an association between cognitive impairment and violence in schizophrenia were collected through a Medline search using the keyword “schizophrenia” combined with the terms “violence”, “cognition disorder” or “cognition”. Reference lists were then searched for additional articles. Violent behaviour in schizophrenia could be considered as a factor that worsen the evolution of the disorder, and should be always detected by physicians. In schizophrenia, literature reported impairment of neurocognitive functions including vigilance, sustained, selective and divided attention, working, procedural, semantic and episodic memory, perceptual representation, inhibition, the executive system and decision-making ability. “Violent” subjects or deined as having psychopathic personality traits showed defects in attention, working memory and decision-making. Schizophrenia patients with a history of violent behaviour showed, in comparison with schizophrenia patients without a history of violent behaviour, impairment of recognition of facial emotions and executive functions. Remediation programs focused on impaired cognitive functions would, first, improve the health of patients with violent behaviour, and second, reduce the risk of violent acting out. To our knowledge, no study assessed whether impaired decision-making impact on schizophrenia patients with violent behaviour." @default.
