- W2076885100 abstract "The relation between hypertension and Alzheimer's disease (AD) correlated disorders has been studied extensively, over the last 20 years though leading to controversial results so far. If high blood pressure (BP) levels measured in middle age have been consistently associated with a higher risk of AD later in life, results are not as clear when BP levels are measured at older ages. Similarly, results of intervention trials aiming at normalising BP levels (in which dementia or cognition is the most often a secondary outcome) are conflicting and the protective role of antihypertensive therapy, especially in older populations, has not yet been demonstrated. In addition, no particular class of antihypertensive agents has demonstrated a strong and unequivocal beneficial effect on cognition. The 3C Study is a prospective population-based study of 9293 individuals that were aged 65 years and older at enrolment (1999-2001). Since inception, participants have had follow-up examinations every two years during which changes in cognitive functions and blood pressure levels have been repeatedly assessed and incident dementia cases have been screened. A subsample of almost 2000 individuals had a cerebral MRI at inclusion from which cerebro-vascular modifications (white matter lesions and silent brain infarcts) and severity of cortical andhippocampal atrophy have been quantified using automated methods. From the 3C study, a review of main findings on the associations between age, blood pressure level, antihypertensive therapy, damages at brain MRI (small vessel diseases and atrophy) and cognitive outcome will be presented. To conclude, though there are no proven strategies to reduce the occurrence of dementia, strong arguments suggest that long-standing exposure to high BP may cause brain damages leading in turn to cognitive impairment. Based on most recent evidence from the literature, and results from the 3C study, a discussion of optimal designs for future studies to demonstrate the impact of antihypertensive therapy to prevent dementia will be provided." @default.
