A review of the literature revealed that neither alternative can be positively identified as best at preventing or minimizing perineal trauma and reducing subsequent postnatal morbidity. The need for good quality randomized, controlled trials had been highlighted as the way forward. It was hoped that the then forthcoming HOOP (Hands On Or Poised) study would identify which of the two was best at reducing postnatal perineal pain and provide the necessary generalized researchbased evidence to reliably inform midwives’ practice. It is widely recognized that the measurement of pain has inherent difficulties. The HOOP study acknowledged that the evidence collected from women in the category of ‘mild’ pain was potentially flawed, but, nonetheless, utilized this same data to provide what was claimed as the ‘significant’ finding of HOOP.
