- W2077878487 abstract "To the Editor: Occasional fecal incontinence is relatively common among older people who are socially and physically active in the community. They may become socially isolated, depressed, and frustrated as a result of the experience. From our geriatric practice in urban Minneapolis, we found and documented 44 cases (all were female ranging in ages from 69 to 99 years), all of whom experienced isolated episodes of fecal incontinence without symptoms and signs suggestive of underlying gastrointestinal pathologies. Most of the incidents occurred about 30 minutes to 1 hour after meals. Therefore, we postulated that the underlying mechanism could be “gastrocolic reflex.” This reflex increases the intraluminal pressure of the colon and overcomes the aged barrier to maintaining continence. Patients were advised to take 1 tablespoon of vegetable fiber daily to form the stool for easier control of defecation, and also to take one tablet of diphenoxylate HC1 2.5 mg and atropine sulfate 0.025 mg (Lomotil) or one caplet of loperamide HC1 2 mg (Imodium A–D) before going out and to repeat this if staying out longer than 4 to 6 hours. This simple and inexpensive approach was successful in all cases in preventing subsequent episodes of fecal incontinence, and patients regained the confidence and dignity needed to resume their social lives. Many factors (anxiety, loose stool, certain foods, inaccessibility to the bathroom, etc.) may play a role in each case. However, regardless of the causes, patients suffer from loss of confidence and may be unable to continue social activities. We want to emphasize that patients frequently do not voluntarily admit to fecal incontinence, and we may miss the true underlying cause (fecal incontinence) that explains a variety of secondary complaints and symptoms." @default.
