Le but de cet article est de présenter une nouvelle approche, invasive a minima, destinée à réhabiliter un édentement antérieur au maxillaire causé par la présence d’une dent incluse. Le protocole consiste à placer un implant sans procéder au préalable à l’extraction de la dent incluse, la dent intercepte alors le trajet de l’implant. Ce protocole, dont les critères d’inclusion/exclusion restent encore à définir, contrevient au concept implicitement accepté de ne tolérer aucune interface implantaire autre que l’interface os-implant. De nouvelles interfaces autres que l’interface os-implant sont créées mais elles ne semblent pas remettre en question le pronostic des implants. Une analyse du risque est effectuée et la réalité clinique en cours, quoique limitée, ainsi que l’évidence histologique à disposition tissent un faisceau d’indications positif en faveur de ce type de protocole. Le recul actuel porte sur une période s’étalant de huit ans à six mois. Avant d’utiliser communément ce protocole non invasif, il est indispensable de le documenter de manière substantielle, il suggère cependant que certains paradigmes établis en implantologie soient encore susceptibles de changement." @default.
