This is a subject that is not normally taught or considered to be a main stream topic in the medical management of the critically ill patient [2]. Studies do exist [3-6], the most comprehensive being Clough's prospective collected data on 638 consecutive patients in intensive care [3]. This paper has the only UK cost analysis available, which showed that prevention of a pressure ulcer costs a mean of £125 and treatment £315 per patient at 1994 prices. Theoretically, alternating pressure air mattresses should be effective [2], but there is only one recent hospital trial to date indicating a reduction in heel pressure ulcers alone [7]. The clinical evidence supports the use of the low air loss and air fluidised bed to be effective [2]. Intensive care patients have Waterlow scores often exceeding 20, indicating a very high risk of pressure ulcer development. The European Pressure Ulcer Advisory Panel (EPUAP Business Office, 68 Church Way, Iffley, Oxford OX4 4EF) recommend a preventative protocol including a simple pressure-relieving mattress for patients at risk [8]. Adequate nutrition is proven to be helpful in treatment [9]. Sensible early introduction of the more expensive sophisticated support beds will be cost and clinically effective when used correctly [2]." @default.
