- W2078486560 abstract "The article entitled “Venous Injury: To Repair or Ligate, the Dilemma” by Drs. Timberlake, O'Connell, and Kerstein (J VASC SURG 1986;4:553-8) interested me greatly. The pioneer work of DeBakey and Simeone1DeBakey ME Simeone FA Battle injuries of the arteries in World War II.Ann Surg. 1946; 123: 534-579Crossref Scopus (708) Google Scholar and later by Rich and Hughes2Rich NM Hughes CW Baugh JH Management of venous injuries.Ann Surg. 1970; 171: 724-730Crossref PubMed Scopus (146) Google Scholar showed that repair of venous injury, either isolated or concomitant with arterial injury, lowered the incidence of postoperative amputation and death. The data by Bower, Federici, and Howard3Bower R Federici V Howard JM Continuing studies of replacement of segment of the venous system.Surgery. 1960; 47: 132-139PubMed Google Scholar in the early 1940s showed that sequelae of venous obstruction causing postphlebitic syndrome occurred from 5 to 10 years after the onset of venous obstruction. According to Dr. Timberlake, their sole criterion for morbidity was swelling, up to 24 months after ligation for isolated venous injury. Their data included no information on venous refilling or ambulatory venous pressure. Today there is clinical as well as experimental evidence confirming that postphlebitic syndrome is a result of venous obstruction and that venous hypertension will cause neuromyopathy, a devastating complication of venous insufficiency.4Taheri SA Heffner R Pollack L Pendergast D Muscle changes occurring with longstanding venous insufficiency.Contemp Surg. 1986; 29: 72-75Google Scholar We have examined more than 82 muscle biopsy specimens and have done 16 electromyographic studies of patients with postphlebitic syndrome. These studies confirm the neuromuscular damage. The average time from the onset of symptoms to commencement of treatment was 84 months in these patients. We believe that repair of venous injury and establishment of venous continuity rather than ligation not only lower the incidence of amputation but also prevent the serious sequelae of neuromyopathy, an important cause of morbidity for this group of patients." @default.
