- W2078549985 abstract "The duration and nature of structural brain alterations prior to the onset of clinical symptoms of Alzheimer's dementia is unclear. To determine if there are regional grey matter volume differences between initially normal aged subjects who later developed MCI or early AD and similar initially normal subjects who remained normal over an extended time interval. In 1999, 146 normal aged subjects underwent structural T1–, T2– and PD–weighted magnetic resonance imaging as part of an ongoing longitudinal study of aging at the University of Kentucky Alzheimer's Disease Research Center. There has been 100% follow up of these subjects, who receive extensive annual psychometric assessment and neurological evaluation. Six subjects had developed mild cognitive impairment (MCI), and 13 early Alzheimer's disease (AD) by the six–year post–scan assessment point (July 28, 2005). Three scans were discarded due to poor quality. Optimized voxel based morphometry was used to determine regional grey matter differences between the 124 normal and 19 cognitively impaired subjects. Custom prior probability templates were used to perform multi spectral segmentation of the scans, from which grey matter density maps were obtained for analysis in SPM2. In a final modulation step, grey matter volume maps were generated by use of the transformation matrix Jacobian. The 19 normal subjects imaged in 1999 who were destined to develop MCI/early AD by six years following the scan demonstrated grey matter volume decreases bilaterally in the parietal lobes, posterosuperior temporal gyrus, and inferolateral and medial temporal regions bilaterally, compared to subjects who remained normal. Right premotor and orbitofrontal decreases were also present. Primary sensorimotor, prefrontal and occipital cortex was largely spared. The pattern of grey matter decrease we observed strongly resembles the pattern of cortical loss known to occur in early AD. This result demonstrates this structural loss pattern is observable in a retrospectively identified group even prior to the development of cognitive symptoms, e.g., MCI. We conclude that cortical structural losses occur in AD prior to the development of cognitive alterations." @default.
