- W2078550086 abstract "Introduction: ST2, an interleukin-1 receptor family member, is upregulated in the setting of cardiomyocyte strain and has prognostic value in patients with acute decompensated heart failure, chronic severe heart failure, acute myocardial infarction, and dyspnea. Previous studies have identified a cut-point of 30 units/mL for prognosis of 1 year mortality in acute heart failure patients. Whether ST2 has predictive value in less sick patients is unknown. Hypothesis: ST2 levels are predictive of all-cause death in outpatients referred for echocardiograms, independent of other clinical parameters and natriuretic peptide levels. Methods: We evaluated 588 patients (mean age 68 ± 12 years) referred for outpatient echocardiogram at the VA San Diego Medical Center. Subjects were divided into 2 groups based on the optimal ST2 cut-point (28.25 units/mL), derived from receiver operator characteristic (ROC) curve analysis with all-cause death at 1 year as the primary outcome. Results: Incident events occurred in 90 patients (15.3%), including 25 deaths (4.3%). Patients with high ST2 levels (n=211, 35.9%) had an increased risk of death at 1 year (Figure), with a hazard ratio (HR) of 2.72 (95% CI 1.22-6.05, p=0.01) on Cox proportional hazard regression analysis. The increased risk persisted after adjusting for age, creatinine, body mass index, race, the presence of systolic dysfunction on echocardiogram, and logBNP levels (HR 2.30, 95% CI 1.01-5.28, p=0.048).Conclusions: ST2 levels are predictive of 1 year mortality in outpatients referred for echocardiograms. The optimal cut-point derived in this cohort, 28.25 unit/mL, is comparable to the previously identified prognostic cut-point of 30 units/mL for sicker patients with acute heart failure." @default.
