- W2078588898 abstract "Over the last two decades, studies in adults have shown a correlation between adverse childhood experiences (ACE) and unfavorable health outcomes (mental health, substance abuse, suicide attempts, high-risk sexual behaviour, chronic diseases, etc.). However, there are few data on the impact of ACEs on the health status of teenagers, especially youths living in custodial facilities who represent an at-risk group. We undertook this analysis with the objective of evaluating if there is an association between the number of adverse childhood experiences and the health status of teenagers in custodial facilities. This is a secondary analysis from a cross sectional health evaluation study of youth in custodial facilities that took place in 2011-2013. A total of 315 teenagers 14-17 y.o. (134 girls and 181 boys) were asked to complete a self-reported questionnaire about their health status/problems/needs and were then evaluated by a nurse/doctor using a comprehensive standardized data collection form/check list. We identified 8 ACEs: (1) monoparental family; (2) violent death of a family member; family history of (3) incarceration, (4) mental health issues, (5) substance abuse; personal history of (6) intrafamilial or (7) extrafamilial physical abuse before 13 years old; personal history of (8) sexual abuse. CHI square analyses were performed between the number of ACEs and some relevant variables. Since 66% of youths came from a monoparental family, we thought that this ACE might be a confounding variable. Thus, we performed two analysis, one including and one excluding monoparental family from the ACE definition. 91% of the youths reported at least one ACE (girls, 95%;boys, 89%) while 48% of them had ≥ 3 ACEs (girls, 58%; boys, 40%). There was a statistically significant difference (p < 0.05) between youths presenting no ACE versus those presenting one or more ACEs: more of the latter perceived their health status as being worse, presented with chronic conditions and more health problems, had functional complaints, were currently taking medication, had mental health disorders, ADHD, were asking for an appointment with a mental health specialist, had attempted suicide, presented sleep disorders, were feeling more aggressive, although we found very little difference for drug abuse and at-risk sexuality. Withdrawing monoparental family from the ACE definition did not changed much of the associated conditions except for more teenagers with a higher number of ACEs having a history of self-mutilation (p = 0.017). The results of the study confirm that a higher number of ACEs is associated with increased mental and physical health problems during adolescence for youths in custodial facilities. This shows the importance of not only asking about ACEs, but to consider their cumulative impact. Especially for teenagers with 3 or more ACEs, services should be intensified in order to alleviate their negative consequences. Even if monoparental families are highly prevalent in this group, including this event in the ACE definition seems appropriate, as in the original ACE study (Filetti & al)." @default.
