- W2078705877 abstract "To determine the outcomes of higher order multiple pregnancies involving a monochorionic pair. The ultrasound database at the Maternal Fetal Medicine Division at Thomas Jefferson University was searched using the term “monochorionic” from 1/01 to 02/05. The ultrasound reports, outpatient and inpatient charts were reviewed. Patients were contacted via telephone for follow up, as agreed at the time of their consultations and IRB approved. Of the ninety patients identified, fifteen were confirmed as higher order multiples (3 or more embryos). Thirteen of fifteen were triplets and two were quadruplets. Eight patients either surgically reduced (7) or had spontaneous reduction (1) of the monochorionic pair. Seven patients were expectantly managed at their request. Of the seven patients with expectant management, three spontaneously miscarried the entire pregnancy at 10, 11, and 19 weeks, respectively. One patient was lost to follow-up at nine weeks gestation. Three patients had preterm deliveries at 23, 30, and 31 weeks. The 23 week infants all died shortly after delivery. The 31 week triplets required positive airway pressure after delivery and are currently doing well. The 30 week triplets were discharged to home on hospital day 37 and/or 38. Of the eight patients that had fetal reduction of the monochorionic pair, four patients went on to full-term singleton deliveries, and all four infants are doing well. One patient is currently 23 weeks pregnant without complications. One patient delivered twins at 34 weeks without complications, and the infants are doing well. Two patients were lost to follow up. Early identification of monochorionicity in higher order multiples with subsequent reduction of the monochorionic pair improves pregnancy outcome." @default.
