However, recent electrophysiological, anatomical, and neurochemical studies suggest the existence of pain facilitory mechanisms as well. These should have environmental triggers, just as do pain inhibition mechanisms. There are multiple pain inhibition systems with different environmental activators, and the authors argue that a parallel situation exists with regard to pain facilitation. The authors propose two different pain facilitation mechanisms: one that is activated by environmental signals for “safety” and involves a descending pathway that opposes opiate action at the level of the spinal cord—an antianalgesia system—and another that is activated by illness and increases pain sensitivity—a hyperalgesia system. The level of pain experienced would result from the balance of activity in analgesia, antianalgesia, and hyperalgesia circuitries." @default.
