- W2079145191 abstract "INTRODUCTION: As the incidence of perioperative stroke has fallen, the use of tests of cognitive or neuropsychological function as a means of detecting neurological injury after cardiac surgery has become widespread. A number of confounding factors, including depressed mood, may impair psychometric test performance. For this reason many investigators have included measures of depression in their patient assessment battery to ensure that any cognitive impairment detected is not merely a manifestation of post-operative depression. The finding that the incidence of new depression following cardiac surgery is low and that postoperative cognitive impairment cannot be explained on the basis of depression alone suggests that measures of depression may not be necessary. [1] In this study we have sought to examine the influence of preoperative mood on the incidence of depression after coronary artery bypass graft (CABG) surgery. METHODS: With Institution Ethical Committee approval and informed consent, 157 patients (mean +/- SD age 59.0 +/- 8.3 years, 138 males) scheduled to undergo elective primary CABG were studied. The Beck Depression Inventory (BDI) questionnaire was administered 1 week before surgery and 1 week and 8 weeks after surgery. A BDI score of 10 or more was used as indicator of significant depression. RESULTS: A total of 155 (99%) patients completed all three assessments. Mean +/- SD BDI scores were 7.4 +/- 4.7 before surgery, and 8.2 +/- 5.4 and 5.1 +/- 4.7 at 1 week and 8 weeks, respectively, after surgery. The number of depressed patients (BDI>=10) was 41 (26.1%) before surgery, and 60 (38.7%) and 28 (18.1%) at 1 week and 8 weeks, respectively, after surgery. One week after surgery 34/116 (29.3%) patients, who were not depressed before surgery, were depressed, whereas 26/41 (63.4%) preoperatively depressed patients remained depressed. Eight weeks after surgery only 7/116 (6.0%) patients, who were not depressed before surgery, were depressed, whereas 21/41 (51.2%) preoperatively depressed patients remained depressed. There was a weak but statistically significant association between increasing patient age and increase in BDI score from baseline at both 1 week (rho=0.168, p=0.036) and 8 weeks (rho=0.300, p=0.0002) after surgery (Spearman). CONCLUSIONS: The incidence of new depression in this investigation was low. Of those patients who were depressed before surgery more than half remained depressed after surgery. We confirm the previous finding that new depression is uncommon after coronary artery bypass surgery and that preoperative depression is present in more than half of those patients with postoperative depression.
