It has been estimated that approximately 5% of the world's population is chronically infected with hepatitis B virus (HBV), and it is anticipated that the number of HBV carriers will reach 400 million by the year 2000.69 HBV infection is the chief cause of cirrhosis and hepatocellular carcinoma (HCC) worldwide.95 Although effective hepatitis B vaccines have been available since the early 1980s, this has not resulted in a major reduction in all high-risk groups in the United States, and efforts to provide routine perinatal and childhood vaccination are just now emerging.1 This further underscores the need for effective medical therapy for patients with chronic HBV infection. Currently, interferon (IFN) α-2b is the only Food and Drug Administration-approved biologic response modifier for chronic hepatitis B. This article summarizes IFN's therapeutic role in this disorder. Guidelines, with respect to patient selection and response to therapy, are provided. Also, the role of IFN in treating atypical cases of chronic HBV infection, such as those with extrahepatic manifestations, decompensated cirrhosis, and combined infection with HIV or HCV, is discussed." @default.
