- W2079499182 abstract "Chung and Feder1 investigated prospectively the effect of corneal thinning after laser in situ keratomileusis (LASIK) on the corneal penetration of topical eye medication. They instilled tropicamide 1% and measured the change in pupil size over time. The authors conclude that faster pupil dilation occurred as a result of corneal thinning after LASIK, but no significant correlation between corneal thinning and pupil dilation was found. The authors claim that “drug penetration after LASIK surgery has not been studied.” However, we have studied pupil response to tropical tropicamide to examine drug penetration in the eye in photorefractive keratectomy patients.2 Our study was similar in design to the Chung and Feder study with 2 important exceptions. We used diluted tropicamide of 0.1% since in a pilot study, we found that tropicamide 1.0% created the maximum effect in all examined eyes and no comparison can be made between eyes. A concentration of 0.1% gives less than maximum dilation of the pupil 30 minutes after instillation and may be used for between-eye comparison. Chung and Feder used a Colvard pupillometer in a lighted room. Different light conditions may alter the effect of dilating drugs in eyes with irises of different colors, especially with regard to the time to reach a maximum pupil diameter. In our study, we eliminated the light factor by measuring pupil diameter with an infrared videopupillography pupillometer (El-Mar series 2000) in a completely dark room. We found a trend not only for faster penetration in thinner corneas, but also for a greater final dilation. These results could not be achieved using tropicamide 1% drops.
