• W2079546042 abstract "Obwohl die arterielle Hypertonie eine weit verbreitete Erkrankung ist mit gut dokumentierter, schädlicher Wirkung auf das Herz und Gefäßsystem, ist die Kontrolle dieses Risikofaktors weltweit nur suboptimal. Zur medikamentösen Therapie steht heutzutage ein breites Arsenal antihypertensiv wirksamer Medikamente zur Verfügung. Erfahrungen aus der täglichen Praxis sowie große Interventionsstudien zeigen, dass eine Monotherapie in vielen Fällen zur suffizienten Blutdruckkontrolle nicht ausreicht. Durch eine frühzeitige Kombinationstherapie lässt sich eine raschere Blutdrucksenkung erzielen, was einerseits vor allem für Patienten mit ausgeprägter Hypertonie oder bereits etablierten Endorganschädigungen günstig ist und andererseits zur besseren Medikamentencompliance der behandelten Patienten beitragen kann. Obwohl die medikamentös erzielte Blutdrucksenkung hauptsächlich für die Risikoreduktion von kardiovaskulären Ereignissen verantwortlich ist, weisen dennoch einzelne Medikamentengruppen positive Effekte auf, welche unabhängig von der reinen Blutdrucksenkung sind und daher eine Kombination von Antihypertensiva sinnvoll macht. Weiterhin können durch eine niedrig dosierte Kombination von mehreren Präparaten Nebenwirkungen einer hoch dosierten Monotherapie vermindert werden. Die Zusammenstellung einer Kombinationstherapie soll sich an den demografischen Merkmalen des Patienten, seinen Komorbiditäten und an den pharmakodynamischen Profilen der Medikamente orientieren. In großen Interventionsstudien hat sich bei Hochrisiko-Hypertonikern insbesondere die Kombination eines Kalziumantagonisten mit einem ACE-Hemmer als vorteilhaft erwiesen. Weiterhin ist die Kombination ACE-Hemmer resp. Angiotensinrezeptor-Blocker mit einem Diuretikum pathophysiologisch sinnvoll, beliebt und in diversen Fixkombinationen verfügbar." @default.
